一、 本案係依衛生福利部國民健康署委託「台灣癌症登記工作計畫」辦理。
二、 依據最新WHO Classification of Tumours of Haematopoietic and Lymphoid Tissues (2017)彙整而成。因2000年出版之ICD-O-3編碼不敷使用，且現行病理報告按最新組織型態記載，故提供最新病理組織型態編碼供醫院使用(附件更新自癌登107001號通知文內容)，WHO 2017年新增編碼請於2017診斷年個案開始適用。注意事項如下：
(1) 新增性態碼為1(非惡性)的血液腫瘤編碼，原ICD-O-3無此類組織型態碼，僅提供參考用，請勿申報。
(2) 新增編碼9715/3 Anaplstic large cell lymphoma, ALK-negative (原ICD-O-3編碼為9702/3)、9715/3 Breast implant-associated anaplastic large cell lymphoma、9749/3 Erdheim-Chester disease、9819/3 B-lymphoblastic leukaemia/lymphoma, BCR-ABL1-like、9877/3 AML with mutated NPM1 (原ICD-O-3編碼為9816/3)、9878/3 AML with biallelic mutation of CEBPA (原ICD-O-3編碼為9816/3)、9879/3 AML with mutated RUNX1、9912/3 AML with BCR-ABL1、9968/3 Myeloid/lymphoid neoplasms with PCM1-JAK2、9993/3 Myelodysplastic syndrome with ring sideroblasts and multilineage dysplasia。
(3) 性態碼3 (惡性)變為1 (非惡性)的血液腫瘤編碼如下：

三、 ICD-O-3組織型態編碼：
(1) 新增8801/3-8804/3 New term，即Undifferentiated (未分化) 描述。
(2) 原編碼9871/3 Acute myeloid leukemia, t(16;16)(p13;q11)有誤，請修正為Acute myeloid leukemia, t(16;16)(p13;q22)。
四、 依癌登中心諮詢病理專家意見：
(1)修正8830/3 Undifferentiated high-grade pleomorphic sarcoma於原發部位為BONE適用；若發生於其他部位，則以8802/3 Undifferentiated pleomorphic sarcoma申報。
(2)有關惡性腦膜瘤個案(M-code 953)，其發生部位應編碼為腦膜(C70 Meminges)。
